  • The Quik-Script Medical Card is not a prescription, medical card or an insurance discount card.  It will not save you money but it just might prevent your doctor or pharmacist from providing medications that conflict with those you currently are taking.  It may even help save your life!

QUIK-SCRIPT Terms and Conditions

QUIK-SCRIPT, ITS PARENT CARBON ENTERPRISES, INC., AND ANY OF ITS EMPLOYESS AND/OR AFFILIATES WILL NOT BE HELD LIABLE FOR MISUSE AND/OR MISINTERPRETATION AND/OR DISREGARD OF THE INFORMATION PROVIDED ON THE QUIK-SCRIPT MEDICAL INFORMATION CARD [hereinafter referred to as the “QS-Card”].  IT IS THE SOLE RESPONSIBILITY OF THE CUSTOMER TO PROVIDE COMPLETE, ACCURATE AND CURRENT INFORMATION TO QUIK-SCRIPT FOR THE PURPOSE OF PURCHASING A QS-Card.  IT IS THE SOLE RESPONSIBILITY OF THE CUSTOMER TO MAKE SURE THE INFORMATION DISPLAYED ON THEIR QS-Card IS ACCURATE, COMPLETE AND CURRENT.  QUIK-SCRIPT, its parent Carbon Enterprises, Inc., and its employees and/or affiliates will NOT be held liable if the information displayed on the QS-Card is incomplete, inaccurate or obsolete.

The purpose of the QS-Card and its displayed information is intended only for use as an informational aid.  The information displayed on the QS-Card is NOT intended to be used as a directive on what treatment and/or assistance the customer may need in any situation.  ANY AND ALL MEDICAL TREATMENT, DIRECTIONS AND ASSISTANCE SHOULD ONLY BE ADMINISTERED BY QUALIFIED AND/OR CERTIFIED INDIVIDUALS.  The customer should only follow the medical directions of qualified and/or certified individuals.  QUIK-SCRIPT, its parent Carbon Enterprises, Inc., and any of its employees and/or affiliates will NOT be held liable if the information displayed on the QS-Card is improperly used by any person, persons or entity as a directive for such treatment in any situation.

IT IS THE SOLE RESPONSIBILITY OF THE CUSTOMER TO ORDER AN UPDATED QS-Card IF THE INFORMATION ON THEIR QS-Card IS INACCURATE, INCOMPLETE OR OBSOLETE.  IT IS THE SOLE RESPONSIBILITY OF THE CUSTOMER TO PROVIDE COMPLETE, ACCURATE AND CURRENT INFORMATION TO QUIK-SCRIPT FOR THE PURPOSE OF ORDERING AND/OR PURCHASING AN UPDATED QS-Card.  QUIK-SCRIPT, upon proper request by the customer, will produce and mail to the customer one (1) updated QS-Card at no charge within twelve (12) months of original date of purchase.  A protective case will not be provided for the updated QS-Card.  After twelve (12) months from date of purchase, the customer may purchase an updated QS-Card for a fee.  The date of purchase is designated as date payment funds from customer were received by QUIK-SCRIPT, not the date the mail-in order form was printed or mailed if purchased offline, and not, necessarily, the date the order was made if purchased online.  If payment of funds was by check, the date of purchase will be the date the customer’s check funds have cleared as sufficient.

The purchase price of $14.95 (USD) for a QS-Card includes one (1) QS-Card and one (1) protective case.  The price of a Duplicate QS-Card or update QS-Card does not include a protective case.  Additional protective cases may be purchased for $1.00 (USD) each.  All prices for any and all items are subject to additional applicable sales tax.  All prices include shipping and handling.

QUIK-SCRIPT may change the design, layout, or categories of information displayed on the QS-Card as we deem necessary.  Images of the QS-Card and/or the protective case used for print or electronic media advertising or promotional materials may not reflect the precise design, layout and/or categories of information displayed by a QS-Card received by a customer.

QUIK-SCRIPT may truncate or abbreviate information provided by the customer in order to produce the customer’s QS-Card so it meets the intended purpose of the QS-Card.  The resulting terms and abbreviations which may possibly be used will be those which are generally accepted for such use by medical personnel and guidelines of various entities such as the AMA, FDA and/or other appropriate entities.

QUIK-SCRIPT is not responsible for any damaged, lost, stolen, and/or altered QS-Card or inaccurate, incomplete or obsolete QS-Card information. 

QUIK-SCRIPT or its parent Carbon Enterprises, Inc. may change any information on and/or about our products, website, advertising and/or promotional materials and/or Terms and Conditions without prior notice.
